Accurate and complete answer sheets help ensure timely score processing. Before returning paper/pencil materials to NOCTI, Coordinators should review all answer sheets and required documentation to verify that information is complete and accurate.
Answer Sheet Requirements
Answer sheets must be
filled out and bubbled completely
and accurately. Incomplete or inaccurate
information on
answer sheets will cause scoring
delays and may incur fees for
corrections.
Before submitting answer sheets, verify the following information is complete:
- Test Taker Name (last, first,
MI)
- Participant
ID
- Test
Code
- Test
Version
- Site
Code
- Demographic
Code
- Title
of
Assessment
The Participant ID must:
- Be unique to each test taker
- Not be reused by another test taker
- Remain the same for both pre- and post-test administrations, when applicable, to support pre/post comparative reporting
Coordinator Cover Sheet Requirements
Coordinator cover sheets are required when returning answer sheets to NOCTI for scoring. It is important to include ALL information on the coordinator cover sheet to ensure timely and accurate score processing.
Include
the following when shipping
materials to NOCTI for score
processing:
- Coordinator cover
sheet
- Completed answer
sheets
- Irregularity reports,
if
appropriate
- Comment sheets, if
appropriate
Do not include:
- Unused test booklets or
answer
sheets
- Scrap paper or test taker skill-based
projects
- Any other
items

In
order to track delayed or missing
packages, shipping receipts should
be
retained until answer sheets are
received by NOCTI. Answer sheets

packaged and shipped separately
from materials being returned for
credit.
