Accurate and complete answer sheets help ensure timely score processing. Before returning paper/pencil materials to NOCTI, Coordinators should review all answer sheets and required documentation to verify that information is complete and accurate.

Answer Sheet Requirements
Answer sheets must be filled out and bubbled completely and accurately.  Incomplete or inaccurate information on answer sheets will cause scoring delays and may incur fees for corrections.

  • Test Taker Name (last, first, MI)
  • Participant ID
  • Test Code
  • Test Version
  • Site Code
  • Demographic Code

The Participant ID must:
  • Be unique to each test taker
  • Not be reused by another test taker
  • Remain the same for both pre- and post-test administrations, when applicable, to support pre/post comparative reporting
