1. Notify NOCTI Immediately 
  • Contact NOCTI as soon as a security breach is identified or suspected.  
  • The Site Coordinator should lead communication with NOCTI and serve as the primary contact throughout the review process.  
2. Gather and Document Key Details 

Be prepared to provide clear information about the incident, including:  
  • What occurred  
  • How the breach happened  
  • When the breach occurred  
  • Which test titles were affected  
  • Who was involved  
  • What actions were taken once the issue was identified  
3. Submit Information to NOCTI 
  • When requested, provide NOCTI with a clear and complete summary of the incident.  
  • Maintain accurate internal documentation related to the situation, including any notes, communications, or actions taken in response to the breach.  
4. NOCTI Review and Investigation 
  • NOCTI will review the information provided and determine whether a formal security investigation is needed.  
  • This process may begin with a request for a written incident summary.  
  • NOCTI management will assess the scope and potential impact of the breach before determining next steps.  
5. Ongoing Coordination 
  • Site Coordinators should be prepared to respond to follow-up questions and provide any additional information needed to support the review.  
  • Clear internal communication at the testing site is also important throughout the process.  
6. Possible Outcomes and Corrective Actions 

Depending on the findings, corrective actions may be required. These may include: 
  • Site probation or suspension  
  • Required use of NOCTI’s online administration system  
  • Removal of affected test scores from the NOCTI database
